Je me rends la semaine prochaine à BKK avant d'aller faire un stage de 7 mois au Cambodge.
J'ai un pb au pied droit qui perdure depuis quasi 2 mois (genre mini ulcer, là ça cicatrise mais très très lentement et c'est violet...) et après analyse, radio etc. le médecin n'a aucune idée de ce que c'est (et moi j'enrage).
Je viens de tomber sur des photos de leishmaniose cutanée sur google image et ça me fait flipper, ma petite blessure y ressemble étrangement, et étant donné que j'était en Asie du SE en août et septembre, ça pourrait coller.
Je retourne chez mon médecin lundi pour lui parler de ma trouvaille, en espérant qu'il me prenne au sérieux et qu'il s'arrange pour me caler une consultation au service de médecine tropicale du CHU de Nantes.
Je pars jeudi, je ne peux pas différer... est-ce que quelqu'un pourrait me donner des infos sur les consultations de médecin à BKK ? De toutes façons, si c'est ça ils seront sûrement plus au point là bas.
Après je m'affole peut-être un peu mais... je n'ai pas envie de me faire amputer au Cambodge...

J'étais 1 mois en Thailande (où j'ai marché nus-pieds à la plage, bien sûr) et 1 mois au Cambodge, je n'ai pas fait la côte, j'ai marché quelques fois sans chaussures, surtout lors d'épisode de pluies où il fallait se déchausser pour traverser des torrents d'eau 😄
Apparemment la Thailande et le Cambodge ne sont pas présents sur la fiche ?
Je suis carrément bête, j'ai pensé à tout sauf à mettre mes assurances au point (je suis en stage donc je suis prise en charge par la structure via la convention, mais mon stage ne commence que le 1er mars). A combien s'élève une consultation "basique" à BKK ??
OUF ! Ca me rassure un petit peu, car là j'avoue je flippe un peu, je n'ai aucune envie de partir avec un truc qui risquerait de se réinfecter... !
Je vais prendre tout ça en compte et en parler avec mon médecin lundi... sérieusement, ça fait quasi 2 mois et je suis passée par :
- tout d'abord je me réveille un matin avec une espèce de cloque; je devais aller chez le doc pour des vaccins, je lui montre en même temps et "oh, bah si ça ne passe pas je vous donnerai une consult' pour un dermato"...ok ...
- la peau du dessus de la cloque (qui n'en était pas une) s'enlève et s'infecte direct', je retourne chez le doc qui fait "oulahalah" et toc, 1 pommade antibio. Résultat: rien, voire pire;
- J'y retourne, là j'ai droit à une "ohlala" bis (avec "on dirait un ulcer"... 'tin, j'ai pas 70 ans !), et 8 jours antibio "tout terrain" : légère amélioration au niveau de l'infection.
- Après, ça a commencé à cicatriser mais très lentement. Du coup, rdv chez la dermato, qui me dit que j'ai sûrement chopé une engelure (j'ai eu le malheur de lui dire que j'avais eu froid aux pieds la vieille de l'apparition de "la cloque"). Après, radio pour voir si il n'y avait pas d'infection au niveau de l'os. Rien. On me demande si je ne suis pas diabétique. Prise de sang (glucose, plaquettes...) : RAS.
Voilà... là j'ai une petite croûte, brousouflée, quasi indolore, avec tout autour sur environ 1 cm de large, une bande carrément violette.
Sympa, non ? J'ai décrit la chose, au cas où quelqu'un aurait eu un truc similaire. Franchement, ça commence vraiment à me soûler... d'autant que ça fait 2 mois que je n'ai pas pu enfiler mes chaussures d'escalade. Pfff...

Le mieux c'est que vous vous rendez à l'hôpital Saint Louis de Paris. Nous sommes Lundi et vous habitez Nantes, c'est un peu compliqué pour vous rendre à Paris avant Jeudi. Il faut absolument avoir une biopsie avant votre départ. On vous fera parvenir le résultat plus tard, par mail.
Si c'est une leishmaniose cutanée, elle va guérir spontanément au bout de 2 à 4 mois.
On fera une biopsie pour analyse anathpathologique et on sera fixé.

Personnellement je ne donnerais pas un avis "médical" par internet, ou photo...pas plus qu'un médecin cohérent qui attendra les résultats des analyses pour te donner le traitement adéquat.
La leshmaniose cutanée guérit en effet spontanément la plupart du temps, mais elle peut évoluer aussi en forme viscérale bien plus tard avec de graves conséquences possibles.
Donc sans pour autant t'alarmer outre mesure, car c'est probablement bénin et/ou curable, il faut impérativement que tu sois fixé et traité en conséquence.
Tu le seras d'ailleurs aussi bien en Thailande qu'en France, si tu n'as plus le temps.
Je programme de péter un gros cable chez mon médecin cet après-midi. Une biopsie devrait être possible au CHU de Nantes avant jeudi. Au moins avec les résultats, je pourrais me faire traiter à BKK... reste à savoir le coût... pffff franchement les médecins... j'enrage sérieux là...

Desolee pour le manque de news, je suis arrivee a BKK entre temps, c est pour ca.
Alors, je suis bien allee voir un specialiste en medecine tropicale qui, au vu des photos et de la plaie en voie de cicatrisation, a fait plusieurs constats :
- il est quasi sur que c est un parasite. Apres, pas du tout sure d une leishmaniose car d une, ce n est pas endemique a la region et de deux, ca ne se presentait pas exactement comme ca (bon, il y a tout de meme des cas particuliers). Ca aurait pu etre tout un tas de parasites, il y en a tellement !!
- pour etre sur (et encore, il m a dit que les resultats ne sont pas a 100 %), c est sur qu l aurait fallu faire une biopsie (il l avait un peu mauvaise quant aux medecins et surtout a la dermato qui assurait que c etait une gelure pffff), mais le probleme d une biopsie avant de partir, c est que ca m aurait bien bien reouvert le pied (ils prennent un bon morceau apparemment), et il ne me conseillait vraiment pas ca avant de partir (et quand je vois l etat de mes pieds a BKK je me dis heureusement que je ne l ai pas fait, en effet !
- en bref, pour lui la plaie etait en voie de cicactrisation. Les parasites de ce genre sont de toute facon voues a ressortir et la il etait quasi sure qu'il etait deja mort et/ou ressorti (surement pendant la phase tres infectee de la plaie). Apres, il m a dit que la cicatrisation prenait pas mal de temps, que c etait normal de passer par des etapes "de couleur" (violet un jour, beaucoup plus rose le lendemain, re violet... ca faisait partie du processus de cicatrisation de ce genre de plaie)
Bon, je resume quand meme pas mal, mais au final il m a assure que cela ne devait vraiment pas m empecher de partir, que c etait toujours a surveiller mais que tout devrait rentrer dans l ordre (la cicatrisation pourra par contre durer encore 1 mois voir un peu plus pour qu il n y ait plus de marque). Je lui ai pose des questions betes du genre : "ca" ne va pas se reveiller au bout de 2 semaines, elle est bien morte la bestiole " et il avait vraiment l air sur de lui.
J ai passe comme 3/4 d heure dans le cabinet, il a regarde les photos (il les a garde dans sa biblio perso d ailleurs), les radios, analyses sanguinesm fait des recherches etc., il ne m a pas prise a la legere... comme la dermato par exemple.
Du coup je surveille quand meme pas mal. Ca n a pas l air de trop mal se comporter avec la chaleur donc je suis assez positive. Et puis j ai pris une super assurance, je suis couverte a 100% frais reels si j ai un pepin !
Voila ! Bref, dorenavant je saurai qu il faudra que j arrive chez le generaliste avec un diagnostique deja en poche pour ne pas qu il mette 2 mois a m envoyer chez le specialiste...

Voilà, à ce jour je suis à Siem Reap. Ca à l'air de cicatriser pas trop mal en fin de compte, la croûte est partie hier, ça m'a un peu picoter aujourd'hui (seulement quand j'étais posée dessus les jambes pliées ou croisées chez les familles, c'était le Nouvel An Chinois 😉 ils m'on gavé comme une oie !!!), mais ça à l'air pas trop mal.
Je suis un peu parano maintenant, par exemple je sens de temps à autre comme des "fourmis" sur le dessous du pied qui gratouillent bien comme il faut... mmm ça doit être la chaleur...

We're about to go on an organized trip to South Africa, including Kruger Park. We were told that Malarone should preferably be taken in the evening at the same time during a meal. But it's hard to know what time we'll have dinner, especially since I've read that in South Africa, dinner is usually around 6 PM.
I'd love to hear from people who've been on organized trips to this destination and could share their experience. We were thinking of taking Malarone around 7:30 PM, assuming dinner would be closer to 7 PM than 6 PM. Also, if the meal ends up being earlier or later than when we take the pill, would a cookie or a piece of bread be enough to take with Malarone?
I’m planning to set off on a "round-the-world" trip/long journey at the start of next year, lasting between 4.5 and 6 months. For now, the itinerary looks like this: South Korea (2 weeks) -> Japan (1 month) -> New Zealand (1 month) -> Argentina (1 month) -> United States (1 month).
I’m on medication (paroxetine 20 mg/day) and was hoping to bring enough for the entire trip so I wouldn’t have to find a doctor on the spot, deal with a molecule that might be slightly different from what’s available in France, or wonder if that’s even a possibility. But after looking into the regulations for each country, it’s suddenly way less simple . Many seem to only allow the amount corresponding to the length of your "stay" in the country... Which is a problem if I arrive in Korea with 4.5 months’ worth of medication, for example.
