Je suis allé me promener 4 fois en Amazonie orientale, je connais un peu.
Cela dépend du coin où tu vas (les centres urbains propres au Brésil sont rarement impaludés), du temps que tu y restes et de la saison. En saison sèche amazonienne (juillet à novembre selon la longitude et les régions), j'ai trouvé moins de moustiques qu'en France en été ...
Un toubib standard, en France, te dira de prendre du Lariam. Tu l'achètes en confiance, prends la 1ère dose et 9 fois sur 10 tu arrêtes aussi vite à cause des effets secondaires; bilan : qq heures (ou jours) de mal-être et qq dizaines d'euros de perdus
Un toubib plus averti en France, te dira de prendre de la Malarone. Tu l'achètes et si tu ne tombes pas ds les pommes après avoir payé la note (tenter de séduire ta mutuelle ...), et que tu le prends chaque jour à même heure, c'est efficace et avec peu (j'ai pas dis pas) d'effets secondaires.
Un mauvais toubib de dira de prendre de la savarine, produit inadapté au palu d'Amazonie
Un habitant local (y compris en guyane française) ... ne prend rien en préventif et va au dispensaire ou à l'hopital se faire injecter de la quinine si crise
Un toubib spécialiste, oralement (afin de ne pas engager sa responsabilité), te dira qu'en dessous de 2 à 3 semaines consécutives en zone impaludée, le bilan risques/avantages incite à ne rien prendre. C'est le délai d'incubation moyen, donc si tu es ds une grande ville (ou en France) avant ou après, et qu'un stique infecté t'a trouvée irrésistible, tu vas à l'hosto, ils diagnostiquent le truc et te piquent (en curatif).
J'ai pratiqué la Malarone 2 fois sur 4, à présent je suis abonné à la dernière formule associé à 3 excellents préventifs bcp moins chers que la Malarone : une GRANDE moustiquaire à suspendre avec anneau central, un spray repellent bien choisi genre 5sur5 ou similaire, et à partir de 17h ... des vêtements à manches longues + chaussettes si le coin craint vraiment

Je viens de téléphoner à un médecin spécialisé ici et il confirme exactement ce que tu écris : Lariam si tu es fauché et si tu ne crains pas les effets secondaires, sinon Malarone. Plus évidemment moustiquaire, répulsif et manches longues le soir.

Je viens de passer quelques jours a etudier le probleme (forums, centre de medecine spécialisée, médecin.. et conscience perso )..
Je pars pour 3 mois et demi, maroc ( pas de probleme ), mauritanie et mali ( zone 2 ) et Senegal (zone 3 ). La majeure partie de mon séjour va se dérouler au sénégal..
J'y ai déjà fait quelques séjours de 2 mois, sous savarine, sans effet secondaires.
Mais depuis quelques mois, c'est passé en zone 3 ( resistant a la savarine ), et donc normalement malarone.. Hormis le cout🤪😠, cela me ferait ingurgiter ça chaque jour pendant 15 semaines a peu pres😕, et le centre de medecine me dit qu'au dela de 3 mois, on ne sait pas trop ce que ça peut donner (pas assez de recul ), donc ils préconisent le lariam pour la durée du voyage plus 3 semaines apres 🏴☠️!!!Bonjour l'alternative !!!
Apres avoir lu tant de témoignages carrement craignos sur les possibles effets du lariam, je pense ne pas prendre de prevention ( sauf bien sur moustiquaire, répulsif, vetements couvrants le soir, spirales insecticides etc..), et emmener une boite de malarone en curatif, au cas ou ( conseil de mon medecin, également vu sur site d'institut Pasteur, et autres ), car je ne serai jamais trop loin d'un centre médical si besoin est.
Ne pas oublier que le traitement curatif ( ou de reserve ) doit etre suivi le plus rapidement possible par une consultation médicale qui permettra de confirmer le diagnostic ( on est pas toubib!!) et si besoin, de compléter le traitement.
Je prefererais la prevention, mais le lariam ça me fait trop flipper !!!

moi je prépares un tout petit séjour en amazonie et j'en suis au même point que toi!!! le lariam après avoir lu tout ce que j'ai lu ce sera non!!!!! un médecin en france m'a d'ailleurs plutôt conseillé le malarone. j'ai rv pour une vaccination chez un spécialiste je vais voir avec lui!!! car je serai de retour en france avt la fin d'incubation pour le cas où sinon je prévois des répusifs deet pour ma peau mes vêtements et un hotel semble-t-il de bonne qualité!!!!

Si c'étais moi, dormant sur le bateau et connaissant un peu le coin :
Je fais l'impasse (et l'économie) sur la prophylaxie (malarone). Désolé, je ne connais pas de pharmacien dévot qui la vend à 50% ...
J'investis un max dans le 5/5 ou similaire (genre 4 ou 5 sprays), dans une bonne moustiquaire, ds des vêtements légers à manches longues, ...
A partir de 16h, je m'asperge (après douche/baignade) de spray sur toute zone découverte de la peau
J'évite de visiter les (rares) coins de village lacustres très infestés (coins où la marée ne nettoie pas assez, où les nids à moustiques se forment)
Pour les fourmis, le 5/5 ou produit d'efficacité similaire (cf sites OMS ou IRD) fonctionne.
Pour le visage, il existe une crème antimoustique (dosage plus faible)
Je n'ai pas souvenir de fourmi ni moustique au milieu d'un bras de l'Amazone.
A toute heure, le spray me semble indispensable lors des balades en pirogue (casquinha) dans les bolons (iguapés et iguarapés).

Tu as fait plusieurs séjours déjà en zone d'endémie palustre. On peut se demander si tu n'es pas déjà immunisé comme la population autochtone? Auquel cas tu n'as plus besoin de traitement préventif.
Par contre je lis souvent des conseils suicidaires de certains grands voyageurs qui sont déjà immunisés et qui n'ont plus besoin de médicament préventif, dire aux nouveaux candidats voyageurs de ne rien prendre.
La malarone est adaptée à la prévention en zone 3. Son inconvénient est d'être cher. Pourquoi ne pas amener avec toi, le Fansidar efficace en première intention et qui coute pas cher du tout? 3 cp en une seule fois, pour moins de 5 euros? Facile à prendre et pas cher en plus. Le Doxycycline est aussi une bonne alternative (dans les zones frontalières de la Thailande avec le Cambodge, le Laos, le Vietnam et le Myanmar).
L'Amazonie est une zone 3 donc beaucoup de résistance aux médicaments antipaludéens, en particulier à la quinine. Donc même si la Malarone est chère, c'est beaucoup moins cher que le visa d'entrée de certains pays !
De toute façon les conseils de protection contre les piqures de moustiques sont toujours bien judicieux. Ca évite de contracter le palu et aussi de se gratter pour certaines peaux sensibles et le moustiquaire imprégné de produits anti-moustiques doit aussi faire partie de ses affaires vestimentaires.

Juste une info sur la malarone, ce n'est pas un antipaludéen trjs très bien supporté. J'ai moi - même été très malade avec ce médicament, j'en ai d'ailleurs fait part dès mon retour de Madagascar à la Veille Sanitaire ainsi qu'au centre de suivi des médicament (excuse je ne me rappelle plus le nom exact). Les symptomes constatés chez moi, avaient déjà été répertoriés sur de nombreux patients. Donc un conseil, si la savarine est valable pour le Brésil, aucune hésitation. J'ai vraiment un super mauvais souvenir de la malarone.
... tandis que moi j'ai déjà pris la Malarone à plusieurs reprises et pendant assez longtemps, sans aucun problème (à part peut-être sur le plan financier 😕 !).
La Savarine ne suffit apparemment pas pour le Brésil, je me suis renseignée auprès d'un centre de médecine du voyage.

We're about to go on an organized trip to South Africa, including Kruger Park. We were told that Malarone should preferably be taken in the evening at the same time during a meal. But it's hard to know what time we'll have dinner, especially since I've read that in South Africa, dinner is usually around 6 PM.
I'd love to hear from people who've been on organized trips to this destination and could share their experience. We were thinking of taking Malarone around 7:30 PM, assuming dinner would be closer to 7 PM than 6 PM. Also, if the meal ends up being earlier or later than when we take the pill, would a cookie or a piece of bread be enough to take with Malarone?
I’m planning to set off on a "round-the-world" trip/long journey at the start of next year, lasting between 4.5 and 6 months. For now, the itinerary looks like this: South Korea (2 weeks) -> Japan (1 month) -> New Zealand (1 month) -> Argentina (1 month) -> United States (1 month).
I’m on medication (paroxetine 20 mg/day) and was hoping to bring enough for the entire trip so I wouldn’t have to find a doctor on the spot, deal with a molecule that might be slightly different from what’s available in France, or wonder if that’s even a possibility. Basically, it seemed simpler on paper...
But after looking into the regulations for each country, it’s suddenly way less simple . Many seem to only allow the amount corresponding to the length of your "stay" in the country... Which is a problem if I arrive in Korea with 4.5 months’ worth of medication, for example.
After all that, and even though I’ll obviously contact the relevant authorities in each country (fingers crossed for a response 😛), here’s my question:

I have several chronic conditions (including asthma and related ones), and I’m planning to travel for a year across different countries.
Generally, I understand it’s possible to travel with approved medications (which should be my case), but often with a limit of 3 months’ worth of treatment.
Since I’m going for 12 months, that means for three-quarters of my trip, I’d have more than 3 months’ worth of medication.
I plan to bring my prescriptions with the INN (International Nonproprietary Name), as well as the original packaging (even though it’s a nightmare, but from what I understand, it’s necessary). But I’m not sure if that’s enough...
