Je pars le 09 avril 2010 et le médecin du centre de vaccination qui vient de nous faire nos vaccins fièvre jaune, nous délivre une ordonnance de Doxipalu pour mon mari et mon fils, alors qu'il me prescrit la Malarone sous prétexte que j'ai les yeux clairs ! 🙂
Je sais que le sujet pour choisir l'un ou l'autre a été déjà évoqué, mais là, je reste sur ma question sans réponse, car même à la pharmacie, ils n'étaient pas au courant !
Qui serait au courant et pourrait m'en dire plus ? ! je vous remercie par avance de m'éviter les pour et contre sur le traitement anti paludéen à suivre, car je ne partirais pas sans l'avoir pris .....!😛

Je te conseille la Doxycycline, qui n'a pas ou très peu d'effets secondaires, mise à part la photosensibilisation de la peau au soleil (peaux claires en général).
En gros, c'est (presque) pareil que le Doxypalu...en moins cher. A l'origine la Doxycycline est un traitement ...anti-acnéique remboursé par la sécu.
Enfin, c'est le traitement utilisé par les militaires français quand ils se rendent dans les zones endémiques, et son efficacité est prouvée, (même si un traitement antipaludéen n'est jamais sûr à 100%).
Le Malarone est beaucoup plus cher: 42 euros la boite de 12 comprimés.
Enfin, et à titre personnel, j'utilise Alaxin Plus vendu localement, compte tenu que la prise (deux comprimés tous les 15 jours) est intéressante et légère pour les gens qui restent longtemps.
Dans TOUS les cas, il est conseillé d'acheter une boite de Co-Arinate (4600cfa) sur place. Ce n'est pas un traitement préventif, mais un curatif à prendre d'urgence si on sent une poussée de palu, sur place ou après son retour en Europe, même en cas de prise de traitement anti palu.

A ma question, mon médecin traitant m'a répondu texto : "Oh vous savez, c'est pas le genre de prescription que j'ai l'habitude de faire.... alors si le médecin que vous avez vu au centre de vaccination vous l'a dit....! faites ce qu'il vous dit !" Et toc ! me voilà bien renseignée... 😉 et c'est pour ça que je fais appel à vos avis.......
Pour info, j'ai aussi décidé de changer de médecin traitant 😕 mais pas que pour cette raison ....!!!!

j'ai les yeux clairs et j'ai pris doxycycline ( antibio que l'on donnait avant doxypalu) il y a quelques temps et pas de soucis, par contre il parait que c'est photo sensibilisant (mais je n'ai pas eu de problème non plus)

Il s'agit donc d'un traitement de seconde intention, et pas à préconiser d'emblée .. (histoire de na pas brûler toutes les cartouches antipaludéennes trop vite ...
Donc, je ne saisis pas du tout cette histoire d'yeux clairs ou pas ... tout le monde aurait du avoir de la malarone sauf exceptions ci-dessus !!!

en terme de résistance aux antipaludéens, le Sénegal est en zone 3 de chloroquino-résistance. les médicaments indiqués en prophylaxie sont la MALARONE, le LARIAM et la DOXYCYCLINE, indifféremment. cette molécule est effectivement un antibiotique qui interfère avec le cycle de transformation des plasmodiae dans l'organisme. elle confère donc une protection, qui n'est pas de 100% contre les accès palustres, notamment les accès graves avec atteinte neurologique. il faut commencer la veille du départ et poursuivre 30 après la sortie de la zone d'endémie. la MALARONE ne se poursuit que 7 jours.
le principal inconvénient de la DOXYCLINE est le risque de photosensibilisation, qui est plus important chez les sujets à peau claire. mais yeux clairs ne signifie pas peau claire, et l'inverse est également vrai : on peut avoir une peau claire et des yeux foncés. il n'y pas de risque au niveau oculaire proprement dit. il est fortement recommandé de se protéger contre le soleil par tous les moyens possibles, notamment crème solaire indice maximal (50) sur toutes les zones exposées. la sensibilisation peut très bien se révéler même si l'on n'est pas sous le soleil d'Afrique.
enfin, DOXYPALU et DOXYCYCLINE ne sont pas "presque" pareils, c'est exactement le même principe actif dans deux médicaments à nom commercial différent. il en existe environ une quinzaine avec les génériques. les indications sont variées, mais le remboursement est différent. ainsi, pour les formes indiquées dans l'acné le remboursement est de 70%, contre 0% contre le palu. le laboratoire a simplement demandé une autorisation de mise sur le marché différente, sous un nom commercial différent.
je terminerai avec la possibilité offerte de changer de traitement en cas de problème sur place (la MALARONE est plus difficile à trouver je pense) si tu pars avec deux traitements différents.
bon voyage et faites l'effort de vous échapper de Saly qui n'est en aucun cas représentative de ce beau pays riche de diversité.

Quel merveilleux voyage j'ai fait ! mais le but de ce sujet de discussion restant le traitement anti palu ! alors je vous donne la suite de mes impressions :
J'ai au final opté pour la doxycycline que le pharmacien a bien voulu me donner malgré l'ordonnance du malarone que j'avais en ma possession. Mon mari et mon enfant aussi ont reçu ce même traitement (pris en charge par la sécu), car le pharmacien m'a dit que Doxypalu n'existait plus et qu'il ne délivrait plus que le générique (Doxycycline coûtant 7,77 euros la boite et remboursé!)....
Arrivés à Saly ....saison sèche, pas un, mais je dis bien pas un moustique à l'horizon ni dans les chambres climatisées......ni le soir ! Nada! rien........ Nous avons attendu de nous trouver en brousse lors d'une excursion voir si les méchants moustiques seraient au rdv !!! Que dalle ! pas un ! et nous avons pourtant tourné en pirogue dans les mangroves !!!! Alors, et c'est là que je vais faire hurler mes amis médecins ! : nous avons arrêté le traitement point barre.
Les Sénégalais rigolent lorsqu'ils nous voient inquiets, et le médecin sur place nous a confirmé qu'en saison sèche, à Saly, le risque était vraiment infime et qu'en plus, dans l'hôtel et ses environs , les traitements anti moustiques étaient réguliers tout au long de l'année (saison des pluies et saison sèche). Par contre, et je le précise, on m'a dit qu'il n'en était pas de même à Dakar ! (??!)..... Donc prudence quand même....!!!!
nous avons pourtant tourné en pirogue dans les mangroves !!!!
les mangroves du Sine Saloum, c'est de l'eau salée. les larves de moustiques ne peuvent s'y développer, les huitres oui..

désolé de te décevoir mais il est loin le temps où les médecins "hurlaient" quand leurs patients arrêtaient leur traitement. la médecine et les médicaments sont devenus malheureusement des produits de consommation comme les autres. en témoigne ton pharmacien qui te délivre de la DOXYCYCLINE sans ordonnance. tu fais une allergie ou un effet secondaire grave et tu peux l'attaquer pour faute professionnelle grave...
ne pas suivre une prescription médicale est un droit, fort heureusement tout s'est bien passé. sache qu'il peut suffir d'une piqûre pour être malade, en particulier chez les enfants. mais chacun prend les risques qu'il estime jouables. prévoir la densité de moustiques en zone impaludée est aussi malaisé que pour les effets secondaires d'un traitement.

Je ne conseille pas aux voyageurs de faire comme moi ! j'ai juste expliqué comment j'avais fait, en mon âme et conscience, tout en connaissant les risques pris, tant au niveau de la prise de médicament sans ordonnance (je n'aurais jamais quoiqu'il en soit poursuivi mon pharmacien, car je suis du genre à m'assumer toute seule, sans faire porter quelque responsabilité sur autrui)! que dans l'arrêt du traitement. Je tenais toutefois à le préciser...... Bien évidemment, je ne suis pas l'exemple à suivre, j'avais peut-être oublié de le préciser et votre message plein de bon sens est là pour me le rappeler ....!

We're about to go on an organized trip to South Africa, including Kruger Park. We were told that Malarone should preferably be taken in the evening at the same time during a meal. But it's hard to know what time we'll have dinner, especially since I've read that in South Africa, dinner is usually around 6 PM.
I'd love to hear from people who've been on organized trips to this destination and could share their experience. We were thinking of taking Malarone around 7:30 PM, assuming dinner would be closer to 7 PM than 6 PM. Also, if the meal ends up being earlier or later than when we take the pill, would a cookie or a piece of bread be enough to take with Malarone?
I’m planning to set off on a "round-the-world" trip/long journey at the start of next year, lasting between 4.5 and 6 months. For now, the itinerary looks like this: South Korea (2 weeks) -> Japan (1 month) -> New Zealand (1 month) -> Argentina (1 month) -> United States (1 month).
I’m on medication (paroxetine 20 mg/day) and was hoping to bring enough for the entire trip so I wouldn’t have to find a doctor on the spot, deal with a molecule that might be slightly different from what’s available in France, or wonder if that’s even a possibility. Basically, it seemed simpler on paper...
But after looking into the regulations for each country, it’s suddenly way less simple . Many seem to only allow the amount corresponding to the length of your "stay" in the country... Which is a problem if I arrive in Korea with 4.5 months’ worth of medication, for example.
After all that, and even though I’ll obviously contact the relevant authorities in each country (fingers crossed for a response 😛), here’s my question:

I have several chronic conditions (including asthma and related ones), and I’m planning to travel for a year across different countries.
Generally, I understand it’s possible to travel with approved medications (which should be my case), but often with a limit of 3 months’ worth of treatment.
Since I’m going for 12 months, that means for three-quarters of my trip, I’d have more than 3 months’ worth of medication.
I plan to bring my prescriptions with the INN (International Nonproprietary Name), as well as the original packaging (even though it’s a nightmare, but from what I understand, it’s necessary). But I’m not sure if that’s enough...

After those 6 months, is it possible (instead of bothering a relative to send the medication) to see a foreign doctor to get another 6-month travel authorization? How does it work at the airport? We, as parents, have been through it in Egypt with pretty bad cases, but we’d really like to avoid that for our daughter. For several years now, increased aggression has been observed in Cape fur seals.

It was recently confirmed that this is due to rabies.
The suspected origin is contamination in Namibia by jackals.
The contagion seems to be spreading.
Preventive vaccination (of the animals) is being considered.

Rabies affects all mammals.
Once symptoms appear—sometimes weeks or even months later—death is inevitable.
It’s the bite that transmits the disease.
There are so-called "furious" forms with aggression, but also forms without aggression.
In case of a bite, in addition to standard wound care, tetanus prevention… rabies prevention is essential.
