Le 9 juin 2011, je suis parti avec mon compagnon pour un tour du monde de 18 mois. Tout se passait super bien jusqu'au mois d'août où j'ai été hospitalisé en Malaisie. Il m'a été diagnostiqué une embolie pulmonaire bilatérale. Après 17 jours à l'hôpital à Kuala Lumpur, nous avons été rapatrié chez nous en Suisse. Notre voyage est donc interrompu depuis août.
Depuis le diagnostique posé, je suis sous traitement anticoagulant (warfarine puis Sindrom) afin d'empêcher la coagulation du sang et donc la formation de caillots. Cependant, mon état étant stabilisé et comme il est théoriquement possible de voyager avec un tel traitement, nous envisageons de repartir au mois de janvier. Nous avons revu notre itinéraire afin de pouvoir effectuer les prises de sang nécessaire pour connaître mon taux INR lui-même nécessaire pour ajuster mon traitement et la prise des médicaments. En gros, nous pensons nous rendre au Vietnam (1 mois), en Nouvelle-Zélande (6 semaines), en Nouvelle-Calédonie (1 mois), en Australie (2 mois) et vraisemblablement aux Etats-Unis (6 semaines). On a garde l'Amérique du Sud pour une autre fois.
Ma question est la suivante : y-a-t-il quelqu'un qui a effectué un voyage de plusieurs mois avec un tel traitement ? J'aimerais avoir un retour d'expérience afin de "m'instruire" et peaufiner ma préparation.
C'est bien beau de faire faire des INR régulièrement mais s'il y a changement, comment allez-vous ajuster votre médicament? C'est très pointilleux, il faut s'y connaître sinon on risque de tout chambouler notre INR et être à risque de caillots ou d'hémorragies. Sachez qu'entre autres médicaments, un traitement de 3 jours d'antibiotique contre la diarrhée, Ciprofloxacin, risque de changer votre INR. Il faudrait aller avec Azithromycine qui est moins pire, ou au mieux consulter votre pharmacien pour connaître d'autres alternatives (biaxin je ne suis pas certaine). Assurez vous d'apporter assez de comprimés pour ajuster si besoin ( donc plus de co.) ou en avoir assez en cas de perte.

Merci pour ta réponse. Oui, tu as raison, j'ai omis de mentionner que bien sûr après chaque test INR je prendrais contact (email, téléphone) avec mon médecin traitant ici en Suisse. De plus, je dois encore en parler avec lui afin de déterminer la procédure et les mesures à suivre. Je sais également que bon nombre de médicaments ont des effets sur le traitement anticoagulant, aussi, j'en parlerai avec mon médecin au moment de préparer ma pharmacie de voyage. Et si en cas de doute ou de besoin pendant le voyage, je contacterai mon médecin en Suisse.

Non, je suis infirmière spécialisée en santé-voyage et travaille au sein d'une pharmacie donc suis pas mal au courant pour avoir entendu les conseils que la pharmacienne donne au clients pour toutes sorte d'interactions et de contre-indications avec les médicaments qu'on leur remet pour leur voyage. Je travaille au Canada. J'adore mon travail, c'est passionnant et les clients apprécient notre expertise. Notre clinique (conjointement avec la pharmacie) est cotée comme parmi les meilleures à Québec. J'en suis très fière.

Dans tous les pays dont vous parlez, il sera possible sans problème de pratiquer ce test sanguin, mais une ordonnance d'un médecin local sera probablement nécessaire. De toute façon, un contact avec un praticien sur place qui soit référent en cas de problème est toujours utile dans votre cas de séjours un peu prolongés.
Partez avec un courrier de votre médecin traitant expliquant (en français et en anglais si possible ) de façon détaillée votre cas et la raison de ce traitement.
Attention au voyage aérien (mais votre médecin a dû vous en parler), et au déséquilibre induit par les changements d'alimentation (surveillance INR à renforcer à l'arrivée dans un nouveau pays)
Interactions médicamenteuses nombreuses, donc bien préciser votre traitement si vous consultez pour autre chose.
Une diarrhée, même banale de type tourista, déséquilibrerait franchement votre INR . Vous ne la risquez un peu qu'au Vietnam, mais quelques précautions de base permettent de l'éviter.

Ajouter des infos sur la société d'assistance à contacter pour la prise en charge des frais (n° contrat, moyens de contact d'urgence, ...) et pour les pros, sur le service à prévenir chez l'employeur en ferait un produit idéal pour celui qui n'est pas trop rassuré à l'idée de se retrouver inconscient après avoir été renversé par une voiture à Bangkok ou ailleurs et qui préfèrerait que les équipes de l'hôpital privé compétent ne perdent pas trop de temps à chercher pour obtenir la garantie de prise en charge financière, chose qu'elle font en général avant tout soin.

We're about to go on an organized trip to South Africa, including Kruger Park. We were told that Malarone should preferably be taken in the evening at the same time during a meal. But it's hard to know what time we'll have dinner, especially since I've read that in South Africa, dinner is usually around 6 PM.
I'd love to hear from people who've been on organized trips to this destination and could share their experience. We were thinking of taking Malarone around 7:30 PM, assuming dinner would be closer to 7 PM than 6 PM. Also, if the meal ends up being earlier or later than when we take the pill, would a cookie or a piece of bread be enough to take with Malarone?
I’m planning to set off on a "round-the-world" trip/long journey at the start of next year, lasting between 4.5 and 6 months. For now, the itinerary looks like this: South Korea (2 weeks) -> Japan (1 month) -> New Zealand (1 month) -> Argentina (1 month) -> United States (1 month).
I’m on medication (paroxetine 20 mg/day) and was hoping to bring enough for the entire trip so I wouldn’t have to find a doctor on the spot, deal with a molecule that might be slightly different from what’s available in France, or wonder if that’s even a possibility. Basically, it seemed simpler on paper...
But after looking into the regulations for each country, it’s suddenly way less simple . Many seem to only allow the amount corresponding to the length of your "stay" in the country... Which is a problem if I arrive in Korea with 4.5 months’ worth of medication, for example.
After all that, and even though I’ll obviously contact the relevant authorities in each country (fingers crossed for a response 😛), here’s my question:

I have several chronic conditions (including asthma and related ones), and I’m planning to travel for a year across different countries.
Generally, I understand it’s possible to travel with approved medications (which should be my case), but often with a limit of 3 months’ worth of treatment.
Since I’m going for 12 months, that means for three-quarters of my trip, I’d have more than 3 months’ worth of medication.
I plan to bring my prescriptions with the INN (International Nonproprietary Name), as well as the original packaging (even though it’s a nightmare, but from what I understand, it’s necessary). But I’m not sure if that’s enough...

The suspected origin is contamination in Namibia by jackals.
